Student Medical Benefit Card

A copy of your student medical benefit card can be obtained through the local DMBA office located at the Laie Shopping Center or by calling their office at (808) 466-4077.

Request for a DMBA Login

1.   Send an email to Andrea Uyehara at auyehara@dmba.com to request for your DMBA login. Please include the following information on your email:

  • Full Name
  • Date of Birth
  • BYUH Student ID Number

2.   Change your temporary DMBA username and password: Log in to dmba.com and change your temporary username and password. This temporary login is only good for seven days. Please change the credentials as soon as you can.

Request Certification of Coverage

Call the DMBA local office at (808) 466-4077 or email Sione Fisihetau at sfisihetau@dmba.com and give her the following information:

  • Full Name
  • Date of Birth
  • BYUH Student ID Number
