Preparing Ahead of Time

One of the first steps to completing your mission application is to have a complete physical. Your missionary application packet has two forms that you will need to bring to your appointment. These forms are the Personal Health History for Missionary Candidates and the Physician's Health Evaluation.

Immunizations, Tests & Costs

A mission physical costs $10 for students, including the physical examination, urine analysis, hemoglobin blood test, and vision eye screening. To complete your mission physical in a timely manner, we suggest that you bring your childhood immunization records and blood type documentation to your appointment.

Immunizations are available through Health Services, and payment is required at the time of service. The cost of the vaccine/test is listed below and is subject to change.

IF YOU HAVE HAD A POSITIVE TB TEST IN THE PAST


If your TB test is positive and you have been exposed to tuberculosis, the Church Mission Department highly recommends that you start preventive TB medications before submitting your mission application. Preventive medications can be obtained free of charge through the Department of Health TB Control Program at Lanakila Health Center, 1700 Lanakila Avenue, Ground Floor, Honolulu, HI 96817 (808) 832-5731.
